ないことには~ない

Unless you do something

ヘルメットを被かぶらないことには、この工事こうじ現場げんばには入はいれない。

You cannot enter this construction site unless you wear a safety helmet.

Explanation

「ないことには〜ない」 means 'unless..., you can't...'. The first part is a condition the second part needs; without it, the good result is impossible. It follows the ない form of a verb (nouns and na-adjectives use 「でない」), and the end must be negative, such as 「わからない」「始まらない」「合格できない」; a positive intention does not fit: not 「先生に聞かないことには、勉強する」. Example: 「全員揃わないことには、会議は始められない」 (We can't start the meeting until everyone is here).

Mixed up with

「ないことには」: stresses that without the precondition, the second part is impossible.

「てからでないと」: stresses the order of steps: 「手を洗ってからでないと、ご飯を食べてはいけない」 (You can't eat until you've washed your hands).

「なければ〜ない」: the plain conditional; flatter in tone.
